Personalized Wellness Plan
The first is a personalized wellness plan. It includes all lab work, testing, consultations, follow up phone calls and emails. We offer a 60 day program and a 120 day program. A personalized plan educates you, guides you and motivates you to make positive changes in your life. A good plan leaves a positive impact on your life and helps you achieve things you may not have been able to attain on your own.
A study by the American Society of Training and Development showed that:
People who merely have a goal, have a 10 percent chance of achieving it
People who create a plan for a specific goal have a 50 percent chance of achieving it
People who have someone to hold them accountable for their goal have a 95 percent chance of reaching their goal
In other words, you can double your chances of success by being held accountable through our personalized plan versus merely having a plan.
Included in the Personalized Wellness Plan program is the following:
- Unlimited emails
- Weekly office visits or phone consults to answer questions
- Laboratory testing—All laboratory tests and retests are included in the program. There are no additional charges or fees for testing.
- Supplements—There is no charge for supplements. Whenever you need new supplements or refills on current recommended supplements, they will be sent to you automatically.
By including all office visits, phone calls, supplements and laboratory tests in an all inclusive format allows you to focus on what matters most, your health.

While the Personalized Wellness Plan offers a much better option for most people, you may choose to see Dr. Chauvin using our traditional wellness plan. We offer all assessments, consults, office visits, testing and supplements on a pay as you go basis. The cost is as follows:

Wellness plans are put together to help you achieve the goal of taking control of your health. We are here to help you every step of the way. The symptoms did not appear over night, nor will they go away overnight.
